What to include

A one line description of what you were doing and what happened instead gets a useful answer fastest. If the tool failed on a specific username, send the username you typed and roughly when you tried it. That is usually enough to reproduce the problem.

If you want a profile removed

Email the same address with the exact Instagram username and confirmation that the account is yours. Once ownership is confirmed the username is blocked and searching it here returns nothing. Setting the profile to private in Instagram's own settings is faster and covers every viewer at once.

Copyright complaints

Formal takedown notices go through the DMCA page, which lists what a valid notice has to contain.

What we cannot help with
